What should be the content of the Complaint?

The complaint SHALL CONTAIN the following information:

  1. full name and address (if known) of the complainant and the person/s complained of as well as his/her/their position/s and office/s;
  2. a narration of the relevant and material facts which shows the acts or omissions allegedly committed;
  3. certified true copies of documentary evidence and affidavits of his/her witnesses, if any; and
  4. Certification or statement of non-forum shopping.
